This residential option provides home and support services by a host home/companion care provider. The host home/companion care provider assists the individual with direct personal assistance with activities of daily living and with facilitating inclusion in community activities, use of natural supports, social interaction, participation in leisure activities and supervision of the individuals’ safety and security
